Summary
- Treatment is indicated in all patients with HIV and evidence of cryptococcal infection (Management Guidelines)[Perfect 2010]
- Preferred regimen is induction therapy amphotericin B liposomal 3-4 mg/kg/day IV plus flucytosine 25 mg/kg PO 4 times daily for ≥ 2 weeks, followed by consolidation therapy fluconazole 400 mg/day PO or IV once daily for ≥ 8 weeks (Table 2)[Perfect 2010; CDC OI]
- Amphotericin B deoxycholate 0.7-1.0 mg/kg IV daily can replace amphotericin B liposomal if risk of renal dysfunction is low and cost is a concern
