- Drug selection must rely on understanding drug–drug and drug–food interactions, convenience, tolerability, clinician’s experience, and goal of preserving treatment options
- Resistance testing of limited use in patients off treatment for some time; clinicians must decide if 2 or 3 fully active drugs can be reliably identified
- In absence of medical records, may be ideal to use drugs in newer classes (and not previously received by patient) in combination with NRTIs, an NNRTI, and/or a boosted PI
