Clinical Implications of HIV Pathogenesis
Summary
- Challenges posed by maintaining lifelong ART have spurred efforts to eradicate HIV infection or control HIV replication in the absence of ART
- A case report describes 1 patient who has achieved a “functional cure” of HIV infection—maintaining health with no risk of HIV transmission despite the interruption of ART,[Hütter 2009] but 2 patients in whom the HIV reservoir became undetectable after undergoing allogeneic stem cell transplantation in the setting of continued ART experienced virologic rebound 3 and 8 months after treatment interruption[Henrich 2013b; Henrich 2014]
- Use of agents that reactivate HIV production in latently infected cells is also under clinical investigation as a potential strategy for HIV eradication, including the HDAC inhibitors, vorinostat and panobinostat[Rasmussen 2013a; Keedy 2009; Archin 2012b; Elliott 2014; Rasmussen 2013b; Tolstrup 2013; Rasmussen 2014; Lee 2017; Garrido 2018a; Garrido 2018b; Li 2016]
